We are an AGPAL accredited, privately owned, non-corporate general practice providing professional and personal care to our patients.

We encourage patients to make an appointment with their doctor for review when requiring a repeat prescription or referral as this is regarded as best practice medicine.

For assistance during normal opening hours, please call the practice, or if urgent, an ambulance (phone 000). If you present directly to the surgery, please alert staff to the urgency/emergency. Please be aware that a doctor may not be present at all times.

Rights and Responsibilities

Herald Avenue Family Practice strongly believes that the relationship between a patient and their health provider is based on a two-way understanding, acknowledging that both have a responsibility for one another.

Behaviour. It is our endeavour to treat all our patients with the utmost respect and courtesy. We ask that this courtesy be reciprocated through adherence to our no tolerance policy for abusive behaviour. Please note that any patients who abuse staff may be asked to leave the premises and may be barred from the practice.

Results. Please note that it is the responsibility of the patient to follow-up on the results of any tests ordered by the doctor. Patients are advised to make a follow-up appointment to discuss test results with the doctor unless otherwise advised, or if unsure to ring the practice for further instruction. Whilst the practice has follow-up systems in place, these are not always foolproof, therefore your assistance and vigilance in following up results will minimise the risk of adverse events in your health care.

Confidentiality. We believe that your privacy and confidentiality is important. It is the policy of the practice to maintain the security of personal health information at all times and to ensure that this information is only available to authorised members of staff.
